Personal care is one of our most important responsibilities with our clients. Connecting Hearts realizes that each of our clients has their own unique needs.
That is why we offer a full range of in-home services, giving you the flexibility of choosing and tailoring a plan to meet these needs.
Our staff is oriented and trained so you always have the same Home Care Assistant to assist in the task you require.
Continuity of care is “Top Priority” in keeping the same Home Care Assistant with your loved one. Our staff gets a permanent schedule that flows month after month until you cancel services. We strive for the right personality to care so your loved one feels safe and secure.
Our flat, hourly rate does not go up based on the amount of personal care a person may require.
We are able to help with simple tasks like getting dressed or more difficult actions like bathing or even total assistance with all personal care needs.
Companionship is essential for our clients. It is proven that social contact enhances ones health and quality of life. Our caregivers provide the social company and engage in conversation with their clients.
Good nutrition provides energy. Many seniors do not have the opportunity to eat a well-balanced meal. Our caregivers are out to enhance the health of the clients they care for.
We prepare hot, delicious and nutritious meals. Many are on specific diets and need to have meals prepared to meet those needs.
Do you remember growing up and coming home from school with the aroma of spaghetti sauce simmering on the stove? The aroma alone made you feel hungry. Well, our staff doesn’t just make whatever they feel like making that day. You tell us what you want! It’s like having your own personal chef!
When you are at this point in life, housework can become difficult. Our services include vacuuming, dusting, sweeping and mopping floors, cleaning kitchens, changing bed linen, straightening all rooms, organizing closets and drawers, and cleaning any interior windows that can be reached without a ladder. Laundry includes washing, drying, ironing and putting things away. Connecting Hearts does not provide outdoor cleaning or maintenance.
Let us help! Don’t become a prisoner in your own home, with Connecting Hearts you can start doing those things you stopped. You can go to your weekly bridge game, visit with some old friends, go to the hair salon or do whatever you want to do.
Clients can choose to stay home and let the caregiver shop for groceries of clients preference, run errands such as picking up prescriptions or going to the post office.
Driving is a symbol of freedom. When we lose the ability to drive on our own this doesn’t mean we can’t get out and do the things we once did.
With a caregiver behind the wheel, seniors can have some independence. Consider us your private chauffeur driving to doctor appointments, the store for some shopping or your favorite restaurant and just taking a drive to enjoy the view.
If you have the car we have the drivers.
Connecting Hearts cannot administer medications. They can assist the client in making sure they are taking the right medication by reading medication labels and reminding the client when it is time to take a dose. Ask us about the system we have put in place to help in this part of care.
We can assist with washing and combing a client’s hair and other grooming tasks to enable them to feel good about their appearance.
It is important for our clients to look and feel their best. Assisting them enhances their well-being and makes them feel positive about their appearance.
Physical health increases ones stamina and energy. We assist our clients as needed with mobility to help them maintain an active lifestyle as possible.
Our caregivers are trained to assist our clients in moving and placing them in the correct posture position. Proper transferring and positioning from or into beds and chairs eliminate pressure areas on the skin. It reduces weakening of the muscles.
Whether one uses a wheelchair, walker, cane, etc a plan of care is designed to help them maintain an active lifestyle and focuses on one’s stamina and energy.
It is embarrassing for our clients to need assistance in toileting and even incontinence care. Our priority is to help them maintain dignity and self-esteem as they receive our help.
There are a number of health reasons why a client may need help in feeding themselves. This can be embarrassing for them; our trained staff assists them in giving them a feeling of independence.
In addition, we make certain that our seniors and other clients stay on their prescribed diets. Examples of special diets are Diabetic diets, Low Sodium diet, etc. By enabling our clients to eat a well balanced meal it provides the energy needed for active living.
Family caregivers can benefit from having a little time off now and then. This service can be scheduled as needed to provide family caregivers time to run errands or have some personal time.
We provide 24-Hour care, 7 days a week. Continuity of care is “top priority” for us. Our Care Manager puts a “team” together that has the skills and knowledge to assist the client. A plan of care is tailored to meet client’s needs. Once everything is in place, our Care Manager monitors the client and caregivers weekly.
Our “Team” gets a permanent schedule that flows month after month until the client cancels services. This permanent schedule includes our caregivers working every other weekend.
This gives the client and family a sense of security knowing their home is not a revolving door of different caregivers coming in. When you call in we will give you more details.
Our caring, compassionate “team” assists individuals who have been diagnosed with a terminal illness. Our goal is to have families spend as much quality time with their loved one as possible.
We realize the emotional effect it has on families and the client. That is why our trained caregivers provide the emotional and moral support as well as providing personal care and companionship.
Financial assistance may be available for home care from the Veterans Administration and programs with various agencies.
